When it's time to go home, we hope that you will look back on your experience at TAMC as having been positive.
When your doctor gives permission for you to go home, he/she will write a discharge order. On the day of your discharge, you will receive information regarding any medication you will be taking, special instructions on caring for yourself at home, and special equipment, supplies, or services you will need. Please ask your nurse or doctor if you have any questions about the discharge process.
Discharge Planning will be provided by Social Services for patients who will have special needs after they go home. Any patient can ask their nurse for a visit from Social Services if they have questions or concerns about their discharge.
Your ride home should be arranged with a family member or friend prior to your discharge. If you have personal belongings with you, please ask your driver to arrive early to assist you. A staff member will bring you to the main entrance by wheelchair to your waiting ride. Please note that a car seat is required before newborns can be discharged. Rental car seats are available; please ask your nurse.
Keep in touch after you get home. If you have any questions about your recovery, we urge you to contact your healthcare provider. You may also be interested in one of the educational programs TAMC offers. Contact Educational Services at education@tamc.org, or 768-4160 for more details. |
